Absentee Ballot Request

You may either download a absentee ballot request form or you may in writing request a form from the clerk.

The absentee ballot request form must be returned to the clerk after it is completed and signed.

The form may not be emailed back to the clerk, but faxed versions are acceptable (874-5070)
